usually pictures are edited with photoshop to look like that but you can also use a free program called photofiltre that lets you edit pictures as well.

Gimp is a free photo editing program, quite similar to Photoshop.
I personally would recommend Photoshop, but if you're not using it for professional means, it's not worth it to buy it.

You take the color picker eyedropper looking tool and grab color from an area close to a blemish or uneven part in your skin tone. Say you have a zit with a red area around it, grab color from an area just outside the red zone because it will have the same lighting.
If you were coloring over a zit on your forehead, you would want to take color from the forehead because the light shines on it, but if you took color from an area around your nose, it would have a different value.
Once you've painted over the imperfections, you get your Burn tool (whatever program you use, look up online to see what the equivalent of Burn is) and go over areas that need to be darkened, like eye lashes, the rim just under your eye, where the bottom eyelashes are, the corners of your mouth, etc. You're going to have to experiment with intensity levels and brush feathering however.

Once you've done your shadowing, get your Dodge tool (again, look to see what the equivalent is for your software) and make the brush large and soft. Go over your forehead, apples of your cheeks, nose, chin, whites of your eyes, teeth, and brighten them up. You have to play with it for a while to get the settings that look the best for each area.

SFX are a completely different ball game. It just depends on what effect you want to add, to be honest. I suggest you look at Deviantart.com for stock smoke graphics, add a gradient of different colors to it, then layer it over top your picture and set the layer to Screen or Multiply, or Color Burn/Dodge, etc.

It's an art, it's not just something you can plop out and say "wee, this took me 30 seconds".
